This form is to be used by an Organisation wanting to apply for a group membership

A separate form is to be submitted for each payee. Each payee will be invoiced separately; One application for each invoice and each subsequent payment

Payment

Once this Application Form has been accepted, an invoice will be sent to the email address supplied, and a paper copy sent to the Invioce Address supplied above. Please contact the club Treasurer with any queries regarding invoicing, payment or any other aspects of your application.
